In an attempt to better process all paperwork requests coming through our office, effective October 1, 2010, PHD will no longer accept forms that are handwritten. For your convenience, ALL forms utilized by PHD, as well as the Nursing Assistant Registry, are available in a fill-in format at our web site.

REMINDER

A registered nursing assistant’s name will not be updated on the registry until official documentation is received in Charleston.

Candidates may verify that their CNA test applications have been processed by using the link on the previous page. If the candidate’s name appears on the posted list, that candidate is expected to present for testing at the appointed time and date. If an admission ticket has not been received on Wednesday prior to the test date, the candidate or sponsor should contact this office.
